This was the first "Backlash" event to take place in the WTX Universe, later replacing the annual late April/May PPV, Extreme Rules in 2014. It was the all-important follow up to early April's big event, WrestleMania XXVIII. Backlash was followed by the final Extreme Rules event in May.
There were nine professional wrestling matches on the event's card, including a Money in the Bank cash-in. Opening the show was a singles match between X-Pac and Rob Van Dam for the newly-renamed Light Heavyweight Championship. Following this was a tag team match between "D-Generation X" Triple H & Shawn Michaels and two members from Evolution, Randy Orton & Batista, for the WWE Tag Team Championships. The third match on the card was a Hardcore match between Brock Lesnar and Rhyno for the Hardcore Championship. Following this was a singles match between Kharma and Beth Phoenix for the Women's Championship. Fifth match on the card was a Triple Threat Double Championship (United States Championship and Intercontinental Championship) Ladder match between MVP, Kurt Angle and The Rock. The first main event of Backlash was a singles match between Samoa Joe and CM Punk for the NXT Championship. Following this was a rematch from Wrestlemania between The Undertaker and Kane in an Inferno match for the Undisputed WWE Championship. The main event of the pay-per-view was another Wrestlemania rematch between Christian and Edge in an Iron Man match for the World Heavyweight Championship.
Five title changes occcurred in the pay-per-view, with the World Heavyweight Championship changing hands twice. Four titles were retained. Rob Van Dam and X-Pac opened the show with a singles match for the Light Heavyweight Championship. Rob Van Dam won the title back after a Superkick to X-Pac and a successful pin. Following this was the WWE Tag Team Championships between Randy Orton and Batista (Evolution), and Triple H and Shawn Michaels (D-Generation X). Evolution won after Batista hit a Batista Bomb and pinned Michaels. Up next was a special attraction Hardcore Championship match between Brock Lesnar and Rhyno. Brock Lesnar retained after utilizing an F5 and following it up with a Backbreaker.
Following this was the Women's Championship between the seemingly unstoppable Kharma and Beth Phoenix. Kharma again retained the title after a huge Splash from the second rope, the same way she defeated AJ Lee at Wrestlemania. Up next was the Double Championship Triple Threat Ladder match between MVP, Kurt Angle and The Rock. The Rock finally ended his losing streak and climbed the ladder to win both championships. Following this was the NXT Championship match and Samoa Joe defeated CM Punk after a huge sideslam.
The next match was the Inferno match for the WWE Championship, and Undertaker and Kane dealt a lot of damage to each other, but Undertaker set fire to Kane first. The main event was the Iron Man match and after 15 minutes the score was tied at 1-1. It then went to sudden death, and Edge won the sudden death fall after getting Christian to submit to the Camel Clutch, thus taking the title to Raw. Following the match, Shelton Benjamin came out and announced that he was cashing in the Money in the Bank briefcase. He managed to take advantage and pinned Edge after a T-Bone Suplex to win his first World Heavyweight Championship, and taking the title back to Smackdown.
